Aged Care On-site Pharmacist
The Aged Care On-site Pharmacist Program (ACOP) places community pharmacists within Residential Aged Care Homes (RACHs) to improve medication safety, reduce harm and support the quality use of medicines. Working closely with aged care staff, GPs and external pharmacies, on-site pharmacists conduct medication reviews, identify risks, provide education and ensure compliance with aged care medication standards. Their role enhances existing pharmacy services and strengthens multidisciplinary care.
Eligible aged care homes include government-funded residential, transition care, Multi-Purpose Services (MPS), and Aboriginal and Torres Strait Islander flexible aged care homes, provided they use or commit to implementing an electronic National Residential Medication Chart within 12 months. Participating pharmacists must complete (or be working toward) an APC-accredited aged care on-site pharmacist training program, with full compliance required by July 2026.
Funding is determined by the number of beds within the RACH, one full-time equivalent pharmacist per 250 beds, allocated in 50 bed increments and can be claimed by community pharmacies or by facilities employing pharmacists directly. Claims are managed through the Pharmacy Programs Administrator (PPA).
