Our After Hours Telehealth Pilot in Residential Aged Care Facilities (RACFs) commences in February 2020 and will run for 12 months.
The pilot will provide telehealth patient consultations to up to eight RACFs across our region during after hour periods when a resident’s usual GP is not available. Where appropriate, this will help to manage the resident within the facility potentially avoiding unnecessary transfer to a hospital emergency department. The resident’s usual GP will receive information about any episode of care provided by the telehealth provider.
The service provider will be My Emergency Doctor, a group of specialist emergency medicine physicians who have experience providing telehealth services to RACFs.
GPs will remain the first point of contact for their patient’s care. Contact with My Emergency Doctor will only occur if the resident's GP is not available and you have stated that you would like your patient to use this service in the after-hours period. As the service is delivered by specialist emergency medicine doctors, GPs who have patients at participating facilities will be invited to make a referral to the service, which will enable the use of Medicare items for the after-hours consultation in the event that you are unavailable.
