Dying Matters: Start the conversation with your patients

April 16 - 22 is the first 'Advance Care Planning Week' health event. While it's difficult to talk about, End of Life Care planning can improve the management of a patient's health conditions and help to keep people out of hospital unless it is absolutely necessary.

Therefore, during Advance Care Planning Week, we're asking GPs to start a conversation about end of life care with your patients. We have produced a GP Kit to assist you. It includes:

  • A 'Dying Matters' poster for your consulting room
  • Advance Care Planning vs Advance Care Directive flyer
  • RACGP Advance Care Planning Position Statement
  • NSW Ambulance Authorised Care Plan template and GP Information

Advance Care Planning documents can be uploaded to My Health Record by patients and can be viewed by their GP or other registered clinicians with permission.
