What is this service?
This free, walk-in service provides short-term healthcare and support if you’re experiencing homelessness, or if you are at risk of homelessness, financial hardship, mental health challenges or domestic, family and sexual violence.
As part of this service, a local general practice provides a GP and nurse and Link Wentworth provides a care coordinator. Together, the team offers immediate care and helps connect you with longer-term health and social support.
