What is this service?
The care finder program is a free service that helps vulnerable older people, who need extra assistance, access aged care services and other community supports. If you have no family or support network to assist you, are unsure where to start or find the system overwhelming, care finders can work with you to get the help you need.
A care finder can visit you in person, at your home or another place you would like to meet, and will talk to you about your situation, help you understand your options, and support you to access the aged care, health and community services you need.
Benefits
Care finders make it easier to get the right support, especially if you are facing challenges like language barriers, isolation, or difficulty understanding the system.
Care finders can help with the following:
- talking to My Aged Care and setting up an assessment
- attending and providing support at the assessment
- finding and choosing local aged care services
- filling out forms and helping you understanding aged care service agreements
- checking in once you are linked with services to make sure everything is okay
- connecting you to other supports such as health, mental health, housing, drug and alcohol services, and community groups.
Care finders work with you at your pace and provide support every step of the way – from understanding your choices to helping you apply for and set up the services you need.
Eligibility
You can use care finders if you:
- older people who need intensive assistance to access aged care and other supports
- have no carer or support person who can help you
- have no carer or support person you feel comfortable with or trust to support you
- are eligible for government-funded aged care
and in addition, you should have one or more of these reasons for needing intensive support:
- have difficulty communicating because of language or literacy problems
- find it difficult to understand information and make decisions
- are reluctant to engage with aged care services or government
- would be in an unsafe situation if you do not receive services
Funding
This service is funded by the Department of Health, Disability and Ageing.
Evaluation of Programs
The Second Report on the Implementation of the care finder Program was released in 2025.
Care finders, Primary Health Networks and referrers strongly support the service with the evaluation demonstrating it is meeting its short‑term goals and progressing well toward longer‑term positive outcomes. The Report identified challenges, however overall found the service to be effective, valuable, and widely supported.
Referrals
You can refer yourself to this service. A referral can also be made by a GP, other health provider, family/carer or friends.
