What we do
We help you deliver high-quality care to older people by funding services for patients and residents, and providing training and practical resources on healthy ageing and residential aged care initiatives and reforms. Our support is free and is tailored to meet the needs of your individual health setting. We can visit you in person or provide support online or by phone.
We can help you with:

- advance care planning training, information and resources
- palliative care information, resources and local referral options
- the General Practice in Aged Care Incentive (GPACI)
- quality improvement (QI) initiatives
- support for GPs with patients in residential aged care homes
