What is this service?

WiseMind provides short-term psychological therapy services for residents living in participating residential aged care homes (RACHs) across the Blue Mountains, Hawkesbury, Lithgow, and Penrith areas. 

The program supports residents who are experiencing mild to moderate symptoms of common mental health conditions, or who may be at risk of developing a diagnosable mental illness within the next 12 months. 

Services are evidence-based and person-centered. Support is delivered onsite at aged care homes by qualified mental health professionals, including psychologists, and mental health nurses. The program focuses on early intervention, practical coping strategies, and improving overall wellbeing.

  • are experiencing low mood, anxiety, or emotional distress 
  • want support early, before symptoms become more severe 
  • prefer to receive therapy onsite rather than travelling to appointments 

The program supports emotional wellbeing, resilience, and quality of life for older people living in aged care. 

Eligibility

You may be eligible if you: 

  • live in a participating residential aged care home in the Blue Mountains, Hawkesbury, Lithgow, or Penrith areas 
  • are experiencing mild to moderate symptoms of common mental illness, or are assessed as being at risk of developing a diagnosable mental illness 

Eligibility is determined in consultation with aged care staff, the resident’s GP and the WiseMind team. 

Funding

This service is funded by the Department of Health, Disability and Ageing.

Referrals

WiseMind services are delivered through multiple providers within participating RACHs. You can refer yourself to this service, or you can be referred by another health provider, family/carer or friend through your facility. A referral can also be made by a GP. GPs can find more information and referral details via HealthPathways.
