What is this service?
This free service, previously known as The Way Back NSW, supports people after they have been discharged from hospital following a suicide attempt or suicidal crisis.
The service provides up to three months of tailored support to help you stay safe, connected and supported during a vulnerable time. Support is delivered by a local team of peer support coordinators with lived experience of suicide or mental health distress, who offer understanding and practical guidance.
Support may be provided in person or over the phone, depending on your needs and preferences.

The Universal Aftercare Service offers compassionate, person-centred support focused on recovery and safety.
You’ll benefit if you:
- have recently been discharged from a referring hospital unit after a suicide attempt or suicidal crisis and need ongoing support
- would like help creating or reviewing a personal safety plan
- need support connecting with services such as counselling, financial support or relationship services
- want assistance accessing or coordinating clinical care during periods of increased risk
The program creates a safe and respectful environment that supports recovery, stability, and wellbeing.
Eligibility
You may be eligible if you:
- have presented to referring units at Nepean Hospital or Blue Mountains Hospital following a suicide attempt (primary eligibility criteria), or
- have presented in a suicidal crisis (secondary eligibility criteria)
- live in the Nepean Blue Mountains region
The service is referral-only and accessed through established hospital referral pathways.

These services are available 24 hours a day.
Funding
This service is jointly funded by the NSW and Commonwealth Governments under the National Mental Health and Suicide Prevention Bilateral Agreement (2022).
Referrals
Referrals can only be made by Safe Haven or participating units at Nepean Hospital or Blue Mountains Hospital following hospital discharge.
