What is this service?
This service provides occupational therapy support for older adults living at home in the Lithgow region.
An occupational therapist visits you at home to understand your needs and develop a personalised plan that supports your safety, independence and daily living.
The service is delivered by Live Well Lithgow and begins with an initial home visit, followed by practical support and recommendations based on your needs.
Support may include:
- assistance with My Aged Care applications for assessment or reassessment
- home safety assessments
- assessment and support for everyday activities at home
- prescription of equipment and aids
- falls prevention education and support
- assessment for home modifications
- seating assessments
- assessment for personal alarm devices
- energy conservation education
- pressure area prevention
- cognitive assessment

Eligibility
The service is available for people who:
- are 65 years and older, or 55 years and older for Aboriginal and Torres Strait Islander peoples
- live in the community or in independent living units / over-55s housing
- live in Lithgow or surrounding areas, including postcodes 2786 and 2758 in the upper Blue Mountains and outer Hawkesbury
People living in residential aged care homes are not eligible for this service.
Funding
This service is funded by the Department of Health, Disability and Ageing.
Referrals
You can refer yourself to this service but a GP referral is preferred. GPs can find more information and referral details via HealthPathways.
