What is this service?
The Mental Health Nurse Incentive Program supports people living with severe or complex mental health conditions through ongoing clinical care and coordination.
This free program is designed for people who may need a higher level of support, particularly during periods when symptoms have a significant impact on daily life.
Mental health nurses can help monitor your mental health, support medication management and connect you with appropriate services and interventions.

Eligibility
You may be eligible for this service if you:
- have been diagnosed with a mental health condition
- are experiencing a significant impact on your daily life, including social, personal or work activities
- have been, or may be at risk of, hospitalisation
- are likely to require ongoing treatment and support over an extended period
Funding
This service is funded by the Department of Health, Disability and Ageing.
