What is this service?
Pharmacy Connector Points are free community-based services you can access through selected pharmacies across the region.
Pharmacy staff provide you with in-person or telephone support to help you find local social and lifestyle activities using the My Health Connector directory.
The service is designed to support you if you do not have internet access or if you would like help exploring what is available in your community.
Staff can help you find activities that match your interests and support you to stay socially connected.

- would like help finding local social or lifestyle activities
- are unsure what services or programs are available in your area
- prefer support from someone in person or over the phone
- do not have access to the internet or need help using online directories

The service focuses on making it easier to access local opportunities and stay engaged in your community.
Eligibility
You may be eligible for this service if you:
- are 65 years and older, or 55 years and older
- live in the Blue Mountains, Hawkesbury, Lithgow or Penrith areas
Funding
This service is funded by the Department of Health, Disability and Ageing.
Referrals
You can refer yourself to this service by contacting the service directly or attend as a walk-in during opening hours. Visit the My Health Connector directory to find a participating pharmacy near you. A referral can also be made by a GP, other health provider, family/carer or friends.
