Free, personalised support to help older people build social connections and stay well in their community.

What is this service?

Health Connectors are trained practice nurses who work one-on-one with you to help strengthen your social connections and support networks as you get older. Health Connectors focus on supporting people who may be vulnerable or at risk of social isolation, helping you find meaningful ways to stay connected to your community.

This program uses an approach called social prescribing. Social prescribing links you with non-clinical support such as community groups, activities, or services that can improve your wellbeing. For example, you might be connected with a local walking group, social club, volunteer opportunity, or another activity that interests you and supports your physical and mental health.

Health Connectors help you stay socially connected, active, and supported.

You will benefit if you:

  • are feeling isolated or lonely
  • would like help finding local social groups or activities
  • are living with a chronic health condition and want additional community support
  • would like guidance navigating local community services

Building stronger social connections can improve physical health, mental wellbeing and overall quality of life.

Eligibility

You may be eligible if you:

  • live in the Blue Mountains, Hawkesbury, Lithgow or Penrith areas
  • are 65 years or older and living at home or independent living
  • are 55 years or older and identify as an Aboriginal or Torres Strait Islander person

Referrals

You do not need a referral. You can make an appointment directly with a participating general practice that offers Health Connectors.

Funding

This service is funded by the Department of Health, Disability and Ageing.

Evaluation of Programs

The Evaluation-of-Social-Prescribing-Models-for-Older looked at how three social prescribing programs, including Health Connectors, helped older adults connect with local activities and improve their wellbeing.

It found that the programs successfully supported many participants by reducing loneliness and boosting confidence, especially when Connectors provided personalised guidance. Some challenges remain such as transport barriers and limited capacity in general practice but overall, the programs delivered positive outcomes.

Referrals
