Allied Health Professionals are encouraged to attend this information workshop to learn about the Health Care Homes program and the use of Shared Care Planning.
What are Health Care Homes?
The Health Care Homes program is a new way of coordinating the health care of people with long-term, complex health conditions. The Health Care Homes program is designed to provide one team to coordinate the care of people with chronic and complex conditions who are assessed as eligible and likely to benefit.
This event will cover the following topics:
- Explanation of the Health Care Homes program
- How allied health professionals can be involved
- The benefits of communication and collaboration within the shared care team arrangement
- How to access and contribute to a Shared Care Plan
